Referral process

Referral Process

 

In order to access St Luke’s services a person will need to have been diagnosed with a serious and progressive illness for which there is no known cure. A patient’s GP or specialist nurse will be able to advise on whether the services available at Clifford House and/or St Luke’s Hospice may be of benefit to them. A referral from a healthcare professional is not necessary for patients and their families to access our services at Clifford House; however, to access our services at Little Common Lane patients will need to obtain a referral from their GP or a healthcare professional.

Referrals to the Integrated Community Team are assessed daily and visits offered depending on each patient’s individual needs, symptoms and complexity. 

Referrals for admissions to the In Patient Centre are discussed at a daily referral meeting. A bed may not be available immediately, but urgent referrals will be admitted as soon as possible.
