What is Aftercare?

Some patients with a serious mental illness are detained in hospital, so that they can receive care.  Patients who are detained are eligible for care and support after they leave hospital.  This is known as aftercare.  Both the CCG and the Council have a duty to provide aftercare to patients who have been detained, once they leave hospital.

The CCG is working with the Council to agree how aftercare will be arranged for patients.  When this work has been completed, we will publish details of the policy on this website.
