Award categories

 

Do you know a general practice nurse or health care assistant that has gone above and beyond or made a difference to you? If so we think they deserve recognition and we encourage you to nominate them for an award.

 

There are six individual award categories highlighting the differing impact general practice nurses and health care assistants can make in delivering high quality care.

 

The categories are:

 

Mentorship and Preceptorship Award

This award recognises a practice nurse that has given inspirational support to individuals who are new or newly qualified to practice nursing and/or to student nurses on placement in primary care.

Innovation Award

This award recognises creativity and enthusiasm for new ways of working. This may include the use of digital tools or collaborative and integrated ways of working with other teams or organisations.

Commitment to General Practice Nursing Award

This award recognises an individual or nursing team, who promotes, support and drives the role of the general practice nurse, deliver peer to peer support and quality care that embodies the best of nursing.

Long Term Condition Management Award (supporting the personalisation approach) 

This award appreciates the valued contribution to patient self-management of long term conditions. Nominees will effectively use resources and initiatives to improve patient experience and quality of life.

Rising Star Award

This award recognises the inspiring contribution a nurse new to primary care (within the last two years) has demonstrated through an enthusiastic approach to improving patient outcomes and the general practice nurse role.

Health Care Assistant of the Year Award

This award recognises a Health Care Assistant (HCA) that consistently delivers a high standard of care, who is proud to be a HCA, an active member of the practice team and who inspires patients to achieve best outcomes.

 

Making a nomination

To make a nomination please download the form below, fill it in and send it back to us either via email to sheffieldccg.pcdns@nhs.net or you can print it and hand it in at your GP practice.

 

If you would like a paper copy of the nomination form posting to you, please email sheffieldccg.pcdns@nhs.net with your name and postal address.

 

You can nominate more than one nurse or health care assistant but you will need to do each nomination on a separate form. Please pick the most relevant category for each nomination.

 

When completing the supporting information for your nomination try to include as much information as possible about the nominee and why they deserve to win the award, where possible use examples to support your nomination

 

Nominations close on 4 December 2018 and shortlisted nominees will be informed by January 2019. Winners will be announced during the Sheffield Practice Nurses PLI event in February 2019.  

 

The winners will be put forward for the Yorkshire and Humber General Practice Nurse Awards 2019 with the result being announced in March 2019.
