Designing new models of care

NHS Sheffield CCG has approved the implementation of a Clinical Assessment, Service, Education, Support (CASES) model for Elective Care in Sheffield.

What does this mean for me?

Currently if your GP refers you to a hospital clinic you go to hospital and are seen in the relevant department, maybe have tests or procedures and are often followed up, then discharged back to your GP.

We are looking at designing new models of care such as having more tests and treatments available in the community and improving support to patients so they can manage their conditions better themselves.

To help us develop the new model we want to hear your views of how local health services run and how you would like services to look in the future.

Further information on CASES

By implementing the new CASES model we aim to:

  • Make sure primary and secondary care are more connected;
  • Ensure patients get the right treatment at the right time, in the right place;
  • Make more investigation and treatment services available in the community;
  • Improve the support to patients so they can manage their condition better, and
  • Focus on the outcomes of treatment for patients, rather than counting the number of times a patient is seen and receives treatment by a health service.
