What is this service?

This is the hospital based outpatient service run by a team consisting of a doctor and specialist pharmacist. This service should not be confused with the chronic pain clinic.

Who is this service for?

People with specialist palliative care needs who are well enough to travel to a hospital clinic.

Indications for referral

  • People with unresolved or complex symptoms
  • People who may need monitoring of symptoms or treatment.
  • People on complex drugs e.g. Methadone.

The service may not be suitable for:

  • People with chronic pain unrelated to current life threatening illness.
  • People in whom curative treatment is not possible, but where there are no ongoing physical or psychological symptoms

Who can refer?

  • GP
  • Hospital consultants or their team members after approval
  • Community Specialist Palliative Care Team
  • Site specific Clinical Nurse Specialist after either GP or consultant’s approval

Where and when are the clinics held?

Clinics are held in three hospitals within the city:

How do I refer?

Urgent referrals (appointment required within next 7 days)

Professionals should telephone the appropriate consultant and fax the referral form or letter to the secretary.

Non-urgent referrals

Professionals should fax/send the referral form or letter  to the appropriate consultant. We try to see all referrals within two weeks of referral.
