The CCG will make an offer of care to anyone who is eligible for continuing healthcare.  We will also offer care to anyone eligible for a joint package of care, although this will be limited to paying for healthcare.  The CCG uses a policy to assist to decide which care to offer and you can access a copy of this policy here.

The CCG has also produce several guidance documents which support this policy.  These address specific services that some people may request.  You can access copies of each of the guidance documents below

The CCG also contracts with specific providers for certain services.  This helps the CCG to ensure good quality care is provided, in line with its policy.  In some circumstances this means that the CCG will usually offer care from these providers, unless there are exceptional circumstances.

How do I challenge a decision about the care I have been offered?

The CCG will look again at any decision it has made about an offer of care, on requests.  Requests can be made in writing to the Continuing Healthcare Team, 722 Prince of Wales Road, Sheffield S9 4EU.  Alternatively you can discuss this with a nurse in the team by calling 0114 3051700.
