There is a fast track assessment process, for people who are close to the end of their life, in specified circumstances.  It may only be completed by a doctor, nurse or other clinician, who has detailed knowledge of the person's needs and experience in dealing with their condition.  Click here for a copy of the Fast Track Tool.
