The CCG has to be sure that anyone being assessed for continuing healthcare or funded nursing care agrees to being assessed.  We also have to ask their permission to share information with other staff.  This means that other health and social care staff can take part in the assessment or help to arrange the person's care. 

Sometimes, the person being assessed may not be able to understand why they are being assessed.  On other occasions we may not be sure whether the person is able to understand or not.  If this is the case, the staff involved have to decide whether the person has the ‘capacity' to agree to be assessed.  This includes deciding whether to share their personal information and to agree to a package of care being provided for them.  You can find further details on this on our information page for health and social care staff, which is here
