In Sheffield we aim to ensure that fair and consistent decisions are made for all people when they are being assessed for NHS continuing healthcare and funded nursing care.  These assessments can be complicated and often take place when a person is at a vulnerable stage of their life.  We hope this information helps to make this process easier to understand. 

These pages apply to people aged 18 or above and young people who are approaching the age of 18.  There is a separate process for children's continuing healthcare and you can find information on this link.
