Under the Freedom of Information Act 2000, I am requesting the following information on clinical pharmacists hired to work in Primary Care Networks in NHS Sheffield CCG:
Please can you provide a list of all PCNs in NHS Sheffield CCG Alongside that list, please can you provide:
1. The patient population of each PCN
2. The number of clinical pharmacists hired to work in each PCN using funding from the Additional Roles Reimbursement Scheme as of 31 March 2020
3. The Agenda for Change pay band for each clinical pharmacist hired by each PCN
Please provide the information by email in the form of an Excel spreadsheet.
