I am writing to request information under the Freedom of Information Act 2000.  

I am requesting the following information:

• The name(s) and direct contact email address(es) and direct telephone numbers of the person(s) in your Medicines Management team who is responsible for the blood glucose meters formulary within the CCG Contact details for formulary can be found here:

• We do not have blood glucose meters on the formulary, they are not prescribable on the NHS, however patients are generally given them from their Healthcare professional. GPs can prescribe blood glucose testing strips (BGTS).
• The date of the next review of your formulary in respect of blood glucose meters

• The endocrine chapter that includes BGTS is due to be reviewed in May 2020 (unless reason to prior review – e.g. new guidelines/ preparations).
• How do you intend to review your formulary (detail to include process, time lines)  
• This is done in partnership with secondary care colleagues, and patient feedback/ use, taking into account things such as; cost, usability, availability, functionality and compliance with national standards.
• Is your CCG is part of a local partnership arrangement for formulary review? 
• We work with secondary care to align preparations across the interface wherever possible. Sometimes, due to various reasons deviation between primary and secondary care may occur.
