Please may I request information for the below queries:

- Which primary care networks exist within your CCG.

- What surgeries make up each of these individual primary care networks.

- What is the main surgery within each primary care network.

- Who is the key person within each primary care network.

Response

As a result of an outstanding issue NHS Sheffield Clinical Commissioning Group (SCCG) has not yet confirmed networks and therefore this information is not yet finalised. SCCG is following the process set out by NHS England Guidance to conclude this process and the information will be available once this is completed.
