We are pleased to respond to your request for information and our response is set out below:

1. Do you employ an older person’s* mental health lead or health lead?   

NHS Sheffield CCG does not employ such a lead person but the CCG has an integrated commissioning relationship with Sheffield City Council and the City Council employs a commissioning manager with responsibility for the provision of services for older people, supported by a project manager with a dementia lead responsibility.  The CCG also has senior contracting capacity

i) If yes, what are their responsibilities? 

The commissioning manager has responsibility for commissioning social care provision for older people and together with the dementia lead, are leading on the development of the dementia strategy and a review of the specification of services provided on the dementia wards.  The CCGs senior contract manager oversees NHS MH delivery through the contract management function

NB. Our contract with our provider of adult mental health services is  a ‘Block’ contract so the figures provided are an indicative apportionment of that total block contract.  Spend on ‘Continuing Health Care’ (CHC), ( a package of ongoing care that is arranged where the individual has been found to have a ‘primary health need’ as set out in the national framework for NHS Continuing Healthcare) is included. Dementia spend is not split by age range so is all included in 19-64 age band

3. Have you applied any CQUINs related to older people’s mental health in each of the last five years? 

No CQUINS were applied.

4. Have you applied any Quality Premiums to older people’s mental health in each of the last five years? 

Yes in relation to IAPT services in 2017-18 and 2018-19

i) If yes, what were they?

Context:  Quality Premium requirements are set each year for CCGs by our regulatory body, NHS England.  

QP for 2014 -15 

There was no quality measure specific to older people’s mental health in 2014-15.  

QP for 2015-16 

There was no quality measure specific to older people’s mental health in 2015-16.  

QP for 2016-17 

There was no quality measure specific to older people’s mental health in 2016-17.

QP for 2017-18 and 2018-19 

The QP for these two years required CCGs to choose from one of three MH indicators where they had the highest challenge around inequalities, in order to address these issues and improve. 

The 3 areas were: 

  • Out of Area placements, i.e. patients who receive care and treatment for their MH problem in settings outside their local area, where this is not indicated clinically and could be deemed inappropriate.  
  • Equality of access to Improving Access to Psychological Treatment (IAPT) services, including improved access for Black and Minority Ethnic people and people aged over 65.  
  • Increasing the number of Children and Young People’s with a diagnosable Mental Health problem who are receiving treatment from NHS funded services.  The CCG chose this as our QP measure for the two years 2017-18 and 2018-19, as this was the areas where data showed we had the greatest challenge and need to improve services.  

There was no other measure which related to older people’s mental health.  

Requirements for a Quality Premium for 2019-20 have not yet been issued by NHS England.

5. What was your total annual spend on activities to raise awareness of mental health issues and/or treatment available for them, in each of the last five years? 

It is not possible to quantify the spend on the awareness activities undertaken since they have been a component of a number of initiatives aimed at our contracted employees and our wider population.  Initiatives have included protected learning initiatives for primary and community care practitioners (encouraging attendance through the offer of practitioner backfill funding), a presence at local workshops and 3rd sector events, positive interaction in the town centre with our population during Mental Health Awareness Week, and service publicity at our Annual Public Meetings which attract attendance from our local population.
