Supporting those living with and beyond cancer

We will improve after treatment support and care, with less hospital follow up where possible and more support closer to home.

Sheffield is part of a regional Macmillan funded programme to support people living with and beyond cancer. This includes agreeing and implementing new pathways after treatment (for breast, bowel and prostate cancer) that better identify individual’s needs and post treatment issues to offer more tailored support. It will include where appropriate reducing hospital follow ups and commissioning additional support in primary care and ensuring the transfer of a detailed treatment summary to underpin this.  Sheffield already has a number of NHS, Local Authority, voluntary sector organisations that support people living with and beyond cancer. We will bring these together in 2016/17 to better coordinate this support.
