Hospital Palliative Care Team

What is this service?

The team provides specialist palliative care support on the hospital wards in Sheffield. They aim to provide:

  • Pain & symptom control
  • Emotional support
  • Spiritual care
  • Carer support
  • Help with planning future palliative care
  • Support with end of life care

When is the service available?

The service is available daily during office hours
An on call service is available at other times for advice.

How are referrals made to the Hospital Palliative Care Team?

Hospital staff may make referrals by faxing the referral form to the number given on the referral form.

Who is in the team?

The Sheffield Teaching Hospitals Palliative Care Team includes specially trained nurses (often called Macmillan Nurses), doctors (consultants and specialist registrars), a pharmacist and has access to other therapists.
