Palliative Referral Process
Referrals to any hospice service may be made by health professionals with the consent of the person’s GP (if they are at home or in a care home) or hospital consultant (if they are in hospital), using the referral form.
Referrals for admission are discussed at a daily referral meeting. A bed may not be available immediately, but the hospice will try to admit urgent referrals as soon as possible.
