What happens if i am screened out but I disagree with that decision?

If a person is screened out and disagrees with that decision, they can ask the Continuing Healthcare Team to arrange a full assessment.  The person will be asked to provide any additional information that they have which suggests they may be eligible for a full assessment for eligibility of adult NHS Continuing Healthcare, and which was not available when the Checklist was first submitted to the CCG. A senior nurse in the Continuing Healthcare Team will review all of the information provided and  decide whether a full assessment should be arranged.
