The Decision Support Tool is used by health and social care staff to help them decide whether to recommend that a person is eligible for continuing healthcare.  The Decision Support Tool is often referred to as the DST and is used to collate and present information from an assessment. Sheffield CCG has developed a policy to help in deciding which care to offer to any person who is eligible for continuing healthcare.  This is called the CHC Policy on the Commissioning of Care.  It sets out all of the factors and issues the CCG takes into account, when deciding what sort of care to offer.  You can find a copy of this policy here
