Do you need advice on managing your skin condition?

The information below is intended to provide support to people who have skin conditions to optimise their self-management and reduce the need for them to make appointments with their general practice and also reduce the number of hospital appointments that may be needed.

In Sheffield, health organisations are working together in an Accountable Care Partnership (ACP) to share responsibility for planning and delivering all health, care and wellbeing services. One priority area is dermatology conditions ‘SKIN’, to provide patients with information and links to key sources of self-management information and support.
