Quality in Care Home Team

 NHS Sheffield Clinical Commissioning Group (CCG) has a role in ensuring that good quality care is delivered in Care Homes and that under performing homes are identified and managed in order to make improvements to ensure safety and enhance quality. 

Working in conjunction with Sheffield City Council (SCC), the CCG’s Quality in Care Home Team (QCHT) is responsible for ensuring that effective processes are in place to provide assurance that Care Homes are delivering good quality and safe services. This is achieved by implementing systems to assess intelligence based on risk and take actions determined by a defined escalation process, in line with the NHS Contract for Care Homes. In addition to this work we also take a more proactive approach to assuring quality by capturing the experiences of people using the service.

The QCHT work across older peoples services, mental health and learning disabilities. The team is multi skilled having a vast range of experience and knowledge in order to provide support and guidance so that improvements are made in line with best practice and legislation.

Enhanced Health in Care Homes Framework

Developments have taken place over the last two years to develop a programme to deliver interventions to the care home population in Sheffield based on the Enhanced Health in Care Homes Framework (EHCH). The EHCH care model aims to support the new models of care approach by facilitating proactive, personalised care and support for individuals both in care homes and those living in the community requiring support.  The EHCH framework was co-developed with the six EHCH vanguards and partners in social care. It is founded on a suite of evidence-based interventions, which are designed to be delivered within and around a care home in a coordinated manner in order to make the biggest difference to the health and wellbeing of residents.

The Sheffield priorities for the implementation of the EHCH are:

  • Supporting the care home workforce
  • Reducing avoidable hospital admissions
  • Increase the use of the digital platform through the use of technology
