Active Ageing, Cancer, End of Life Care and Long-term Conditions Portfolio

Our Strategy and Aims

  • To work together for healthier lifestyles
  • To help people to manage their conditions and live better
  • To promote person-centred care
  • To support recovery and enable independence
  • To improve End of Life Care

Services will move from being disease specific, to being integrated, holistic, patient centred, flexible, responsive to needs of both patients and carers, co-ordinated and closer to home. Our current priorities include:

Respiratory

A range of projects to improve diagnosis, management and treatment and patient education

Person Centred Care

Key to delivery of the ‘Care Outside of Hospital’ Programme, we are working hard to promote and embed patient activation, self-management, shared decision making and care planning across GP practices and neighbourhoods to benefit patients all over Sheffield

End of Life Care

Projects to support further development of care for patients at the end of life, both through specialist palliative care services and through training and development of generalist staff

Age Better in Sheffield

A five year Big Lottery funded programme to reduce social isolation and loneliness in older people in Sheffield. The CCG is one of the core partners. For more information, see http://www.agebettersheff.co.uk/

Cardiovascular Disease

Programmes include the ‘National Diabetes Prevention Programme’ for people at risk of developing type 2 Diabetes and the ‘Diabetes Treatment & Care Programme’, delivering improved patient outcomes in four clinical areas. Also,  work on stroke prevention and improvements to stroke and neurological rehabilitation pathways

Care Outside of Hospital’

The focus is on transforming the health and care system outside hospital to enable more people to live well in their community

Andrew is also the CCG's Caldicott Guardian and will champion the importance of respecting the confidentiality of patient identifiable information, as well as supporting the Caldicott Guardians of our member practices.
