Service Description

The Evening and Night Nursing Service is based at the GP Centre, Northern General Hospital, and provides a city wide service to enable nursing care to be delivered over a 24 hour period in patients’ own homes or chosen setting. It is a responsive (not emergency) service and is not GP attached. Visits are prioritised and the service aims to visit within 2 hours. The service operates 17.00 -8.00 hours, 365 days per year.

Staffing

The service has a staff of Qualified Nurses and Health Care Support Workers.

Patient Referral Criteria

All core and enhanced community nursing services including support for level 2 care; and Community Assessment & Rehabilitation Team (CART) patients who have been assessed and referred by the CART team. Staff can refer for an evening or night visit by faxing patient details and care plan through to the service. Patients can phone the service through the evening or night if they have an urgent nursing need.

Links to other services

The Evening and Night Nursing Service works closely with Out of Hours Services, including Intensive Home Nursing, GP Collaborative, Emergency Care Practitioners, CART and daytime nursing services.

Notes

The service has access to basic dressings but do not carry medication. All patients should have dressings and essential items prescribed by a GP or a nurse prescriber which needs to be accessible in the patient’s home.
