Freedom of Information Act 2000 – Request for Information

We are pleased to respond to your request for information and our response is set out below:

Request /Response

I am writing to request information under the Freedom of Information Act 2000.  Please can you supply the following information:

  • What alternative arrangements for Structured Diabetes Education programmes have been commissioned/put in place by the CCG to support people living with diabetes due to the COVID-19 pandemic (e.g. virtual delivery via video conferencing tools, digital etc)?

Diabetes education (DAFNE and DESMOND) is commissioned from Sheffield Teaching Hospitals and is traditionally delivered in group settings. This had to be stopped during the first phase of lock down but services have now resumed for smaller groups with appropriate social distancing in place. The online form of both education programmes (for Type 1 & 2 diabetes) have been offered to patients as they became available.

  • With the recent NHS primary care reorganisation, which organisation (if not your organisation) within your STP/ICS footprint is responsible for the commissioning of Structured Diabetes Education programmes for people living with diabetes?

NHS Sheffield CCG

Could you provide contact details for the main person responsible for commissioning Structured Diabetes Education programmes for people living with diabetes?

Please contact the lead of Long Term Conditions at Sheccg.ltcportfolio@nhs.net
