Request

I am writing to you under the Freedom of Information Act 2000 to request a list of all Primary Care Networks (PCNs) and their member GP Practices.

Response 

This request was made to the South Yorkshire and Bassetlaw Sustainability and Transformation Partnership and forwarded to NHS Sheffield CCG for response (for Sheffield only). Other CCGs within South Yorkshire and Bassetlaw will need to be contacted for their areas.

Please note that from 23rd November the North 2 PCN will be known as the Foundry Network.
