Sheffield Health and Care Partnership

NHS Sheffield CCG is a member of Sheffield’s Health and Care Partnership (HCP), which brings together seven partners in the city to focus on issues that are better addressed collectively.

The partners are: Sheffield City Council, NHS Sheffield Clinical Commissioning Group, Sheffield Children’s NHS Foundation Trust, Sheffield Teaching Hospitals NHS Foundation Trust, Sheffield Health and Social Care NHS Foundation Trust, Primary Care Sheffield Ltd and Voluntary Action Sheffield.

The Sheffield HCP partnership vision is for our health and care services to be joined up, and seamless; to reduce and remove health inequalities in a way that involves people, their experiences and our communities at the centre. A video describing the vision of Sheffield HCP can be viewed here.
