Please can you answer the following questions regarding the APMS contracts.

1. Please can you confirm how many APMS contracts you are currently operating.

NHS Sheffield Clinical Commissioning Group currently has in place two APMS contracts for the provision of primary medical services.

2. Which areas do these APMS contracts cover.

The catchment area defined within both contracts is co-terminus with the boundaries of NHS Sheffield Clinical Commissioning Group.

3. Please can you provide the details of who the incumbents for these APMS Contracts are.

The two practices covered by APMS contracts are:

• The Clover Group;

• Clover City Practice.

4. Please can you confirm the start date of your current APMS contracts.

The commencement date of both APMS contracts is 1^st April 2016.

5. Please can you confirm the date your current APMS contracts are due to renew.

The expiry date of both APMS contracts is 31^st March 2021.
