• How many people are living with Type 1 diabetes in your CCG? (Total number)

In March 2019 there were 2,579 registered patients with Type 1 diabetes 

• How many people with Type 1 Diabetes in your CCG use continuous glucose monitoring ( CGM) (Total number)

Not known, we do not routinely hold this information.  

• Does your CCG have a policy on the funding of CGM? (Yes/No)

No. 

• If your CCG has a policy in place for CGM, please provide a copy of the policy or a link to the policy

• How is CGM currently funded within your CCG? (e.g. routinely commissioned/ routinely commissioned within the scope of the NICE guidance/Individual Funding Request/Patient self-funded/ Hospital funded/Other (please describe) etc.)

• How many IFR applications were received between 1st April 2018 and 31st March 2019 for CGM? (Total number)

Less than 5 were received. 

• How many people with Type 1 Diabetes in your CCG use Flash Glucose Monitoring? (Total number)

In March 2019 there 334 patients prescribed Flash Glucose Monitoring (FGM) via Sheffield Teaching Hospitals and 80 patients prescribed FGM in primary care. 

• Does your CCG have a policy on the use and funding of Flash Glucose Monitoring? (Yes/No)

Throughout the period 1st April 2018 and 31st March 2019 we had the attached position statement in place (agreed Feb 2018). This was updated in April 2019, see below.
