Freedom of Information Act 2000 - Request for Information

Request 

I'm conducting some research into the PCNs and I was wondering if I could get some information regarding the PCNs in your CCG:

• Their names
• Their clinical directors
• How many patients they care for
• A list of the GP surgeries within them

Any information you could provide would be of great help.
