I am writing to make an open government request for all the information to which I am entitled under the Freedom of Information Act 2000.
Please send me:
The number of GP Surgery closures across Sheffield over the past five years.
If you could provide the number of closures each year for 2015, 16, 17, 18 and 19 and the total of GP’s left at the end of each year please.
Response
NHS Sheffield Clinical Commissioning Group (SCCG) assumed delegated responsibility for commissioning primary medical services on 1 April 2016. For information before this date you may wish to contact NHS England if you have not already done so. For years 2016 and onwards please see table:
